Collaboration with medical, psychiatric and other service providers and 24-hour crisis support are included. Participants are adults and older adolescents who are living or planning to live independently within the next six months, with a history of psychiatric hospitalization or other involvement with human services or law-enforcement demonstrating the need for assistance to achieve stable and independent functioning. Activities include education, modeling, direct support/assistance, skills practice, advocacy, care coordination, and individualized service planning. Medicaid, self-pay, or other funding sources are accepted. (Private insurance does not usually cover this service.)
